A leading healthcare provider is looking for a committed Practice Nurse to join their team part-time. This role involves delivering essential patient care, including immunisations and chronic disease management, while collaborating with a multidisciplinary team to improve patient outcomes. This is an exciting opportunity for a well-qualified, enthusiastic and committed Practice Nurse to join our team part time. We have a highly skilled nursing team consisting of Practice Nurses, Nurse Associates and HCA's.
We are looking for a Practice Nurse to work Monday, Thursday and Friday 09.00-17.00.
As a Practice Nurse, you will play a key role in providing essential patient care within our local community. Your responsibilities will include:

Gardenia and Marsh Farm Practice has a list size 13,900 and sites within close proximity one another. The leadership team consists of three partners, Practice Manager, Senior Admin members of the experienced nursing team.
The following are the core responsibilitiesof the Practice Nurse. There may be, on occasion, a requirement to carry outother tasks. This will be dependent upon factors such as workload andstaffing levels:
a.Tobe instrumental in the support of the Infection Prevention Control (IPC) leadand to work to the IPC requirements by providing audits, training, supportand evidence of compliance to the practice manager
b.Tosupport in the publication of the IPC annual statement in conjunction withthe practice manager
c.Todevelop, implement and embed an effective care management programme for thefrail and housebound patients registered at the practice
d.Toassess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practice
e.Todevelop, implement and embed health promotion and wellbeing programmes
f.Toimplement and evaluate individual treatment plans for chronic diseasepatients that promote health and wellbeing
g.Toidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients health
h.Asrequired, to provide routine nursing care to patients in accordance withclinical based evidence, NICE and the NSF
i.Asrequired, to diagnose and manage acute and chronic conditions
j.Providewound care (ulcer/Doppler, etc.) to patients
k.Toreview patient medications to enhance compliance
l.Toprovide guidance to patients in the use of prescribed and over-the-countermedications regarding side effects and dosages
m.Toco-ordinate the provision of travel medicine services
n.Torequest pathology services and process pathology results as required
o.Tooversee the provision of chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as required
p.Tomaintain accurate clinical records in conjunction with extant legislation
q.Toensure SNOMED CT codes are used effectively by all members of the nursingteam
r.Toensure the appropriate maintenance and use of chronic disease registers
s.Todevelop, implement, embed and participate in well woman clinics
t.Todevelop, implement, embed and participate in well man clinics
u.Toensure compliance with the practice chaperone policy
v.Toassist GPs with minor surgery when required
w.Toprioritise health issues and intervene appropriately
x.Tosupport the team in dealing with clinical emergencies
y.Torecognise, assess and refer patients presenting with mental health needs
z.Toimplement vaccination programmes for adults and children
aa. To support patients in the use oftheir prescribed medicines or over-the-counter medicines (within own scope ofpractice)
bb. To liaise with externalservices/agencies to ensure the patient is supported appropriately(vulnerable patients etc.)
cc. To delegate clinical responsibilitiesappropriately (ensuring safe practice and the task is within the scope ofpractice of the individual)
dd. To support the clinical team with allsafeguarding matters, in accordance with local and national policies
ee. To understand practice and localpolicies for substance abuse and addictive behaviour, referring patientsappropriately
ff.Todeliver opportunistic health promotion where appropriate
Secondary key responsibilities
In addition to theprimary key responsibilities, the Practice Nurse may be requested to:
a. Develop and review audit protocols andprocess for the practice, effectively utilising the audit cycle
b. Develop, review and implement nursingprotocols in conjunction with the partners
c. Support all members of the nursingteam, providing guidance when necessary, conducting appraisals and trainingneeds analyses as required
d. Participate in local initiatives toenhance service delivery and patient care
e. Support and participate in sharedlearning within the practice in order to improve patient care
f. Continually review clinical practices,responding to national policies and initiatives where appropriate
g. Participate in the review ofsignificant and near-miss events applying a structured approach, i.e., rootcause analysis (RCA)
h. Coordinate the nursing team rota,ensuring sufficient staff are available to meet patient needs
i. Lead the planning and implementationof changes within the nursing team, enhancing the provision of services topatients
j. Support the practice team with thereviewing and implementation of practice policies and protocols, ensuringconformance to extant legislation
