Job Description for Practice Nurse

Join the multidisciplinary team at Three Valleys Hospital, caring for people with complex mental health issues in a safe and supportive environment. This role involves leading and coordinating all aspects of the physical health needs of service users, supporting colleagues, and promoting teamwork with GPs and other health departments.

Responsibilities include:

  1. Monitoring observations and assessments to identify actions needed and documenting them.
  2. Adhering to the NMC Code of Conduct and ensuring compliance with local, Trust, and national standards.
  3. Developing and supporting evidence-based physical health interventions.
  4. Engaging in practice development, evaluation, and audit activities relevant to physical healthcare.
  5. Collaborating with primary health services and maintaining links with GP practices and other departments.
  6. Monitoring and maintaining Physical Health Dashboards.

Qualifications and skills required:

Work environment:
