A healthcare organization in North London is seeking an experienced Practice Nurse to join its dynamic team. The ideal candidate will have at least 1-2 years of nursing experience in General Practice, focusing on managing chronic diseases. This part-time role offers the opportunity to contribute to patient care in a forward-thinking environment.
We are looking for an enthusiastic PracticeNurse to join our Nursing Team in our busy North London General Practice.
We are a forward thinking practice with 18,000 patients operating over 2 sites. Ourmultidisciplinary team is developing and expanding our services into new waysof working, which include expanding our Nursing Team to take on management ofchronic disease, as well as new ways of working across our local Primary CareNetwork.
Ideally we are looking for somebody withat least 1-2 years nursing experience in General Practice.
We are a training practice and supportteaching of medical students as well as trainee nurses. We are also a part ofa local Primary Care Network of other GP practices and are working to developnew and innovative ways of providing quality care for our patient population.
We have 5 partners, 9 salaried GPs, 3Nurses, 1 HCA, 4 pharmacists, pharmacy technician, dietitian,physiotherapist, social prescriber and a mental health practitioner

The post holder isresponsible for the delivery of general practice nursing to the whole practicepopulation. The focus of the role is a) the delivery of evidence-based practicefor patients presenting with long term conditions and b) the provision ofpreventative health care to the practice population and acting within theirprofessional boundaries. Scope and purpose of therole:
Todeliver a high standard of patient care, as part of the multi-professionalteam, utilising clinical skills grounded in contemporaneous evidence
Tomanage a clinical caseload, dealing with presenting patients needs in aprimary care setting
Toprovide care for those with chronic diseases, including planning, managementand appropriate follow-up
Toengage in and develop nursing services alongside the nursing team and partners
Key responsibilities
Assess,plan, implement and evaluate treatment/management/interventions and care forpatients with chronic disease and long term conditions.
Instigatenecessary diagnostic tests and investigations in conjunction withmulti-professional team.
Assesspatient needs from a holistic view, including physical and psychosocial needsand plan care accordingly
Workalongside other health care professionals to identify, diagnose, monitor andmanage treatment plans for patients at risk of developing long-term conditionsas appropriate
Reviewmedication for therapeutic effectiveness, appropriate to patient needs and in accordancewith evidence-based practice, national, local and practice protocols andformularies
Workwith patients and their carers in order to support compliance with andadherence to management plans
Promotehealth and well-being through the use of health promotion, health education,screening and therapeutic communication skills
Implementand participate in vaccination an immunisation programmes for both adults andchildren
Advisepatients with general travel health advice, support and administer vaccinationswhere appropriate for patients travelling abroad
Supportand manage patients presenting for wound care, in accordance withevidence-based practice, national, local and practice protocols and formularies
Communication:
Utiliseand demonstrate appropriate communication skills to ensure patients are fullyinformed and are able to consent to treatment
Communicateeffectively with patients and carers,recognising the need for alternative methods of communication and takinginto account cultural background, preferred method of communication anddifferent levels of understanding
Communicateeffectively with members of the healthcare team, including external agencies
Attendand contribute to practice meetings

Workcollaboratively within and external to the practice, including the localnetwork to assist in providing new and innovative systems of providing patientcare, including health and wellbeing, as well as assisting in evaluating suchinitiatives
Supportand participate in shared learning across the practice and wider organisation,including participation in significant event, complaints and safeguarding meetings
Leadership:
Act as an advocate in the delivery ofPractice Nurse services to patients, ensuring that the needs of thepatient are the priority
Take responsibility for own developmentlearning and performance including participating in clinical supervisionand acting as a positive role mode
Participate in planning and implementingchanges within the area of care and responsibility
Contribute and participate in the developmentof local guidelines, protocols and standards
Provide nursing representation in the localnetwork, providing guidance and education to network practices asrequired, particularly in relation to health and wellbeing
Teamworking Role:

Contributeto the development of the nursing team
Participatein supervision and mentoring of junior staff, GP registrars, medical/nursingstudents and administrative staff

Participatein the development of clinical protocols, guidelines and standards inconjunction with the healthcare team

Helpthe practice operate in a cost-effective manner
Participatein weekly practice meetings and other relevant meetings
Workalongside the multi-professional team in improving systems formonitoring/measuring performance against quality targets
Ensurepractice policies are implemented and maintained/updated
Management of Information:
Usetechnology and appropriate practice agreed software to aid planning,implementation and monitoring of care
Keepaccurate and timely records as episodes of patient care to ensure quality andaid accurate information retrieval for monitoring and audit processes
Participatein quality searches and presentation of clinical data to aid qualityimprovement
Ensureaccurate and confidential completion and transfer of patient information
Befamiliar with and act in accordance with NMC record keeping Guidance for Nursesand Midwives
Learning and Development:
Participatein annual appraisal
Assessown learning needs and undertake learning/continuing professional developmentas agreed in annual appraisal and in conjunction with the Nurse Lead. Personaldevelopment will be encouraged and supported by the practice and should be inaccordance with practice requirements
Remainup to date with professional/clinical developments to inform safe and effectivepractice
Participatein education and training of all members of the healthcare team including GPregistrars and medical/nursing students
Bewilling to develop your own role in line with new developments within thepractice. If expansion of the current role is required, full training andsupport will be given
Ensurerelevant professional development needs are met to meet NMC requirements- thepractice will support these
Supportlocal networking and attend local educational forums as appropriate
