A modern primary care practice in North Blackpool is seeking an experienced Practice Nurse who will be responsible for delivering high-quality clinical care and patient education. The role offers flexibility in hours and requires a Registered General Nurse with current NMC registration. Key responsibilities involve chronic disease management, health promotion, and collaboration with other healthcare professionals.
The practice is looking for an experienced, motivated Practice Nurse to joining our friendly team. The practice nursing team play a key role in delivering high-quality, patient-centred care within a primary care setting.
The successful candidate will play a vital role in the provision of high-standard clinical care. This role offers some flexibility with hours up to full-time for the right candidate. Salary dependant on experience.
The Practice Nurses work collaboratively within the wider healthcare team to support the reduction of health inequalities, improve patient outcomes and provide comprehensive patient care.
North Shore Surgery is a large, modern, forward thinking, training practice based in North Blackpool. We have 5 GP's, 3 Advanced Practitioners, 5 skilled Practice Nurses as well as HCA's, Practice Pharmacists and a Paramedic.
We also benefit from being part of the North Primary Care Network, sharing further skills in our team of care co-ordinators, first contact practitioners, pharmacists, social prescribers, GP Assistants and a Digital Lead.
Working with the current team the successful candidate will provide clinical care, health promotion, disease prevention and patient education. The practice supports individuals to manage their health and well-being effectively. The key responsibilities of the role will include, but not limited too:
Deliver evidence-based nursing care in line with national guidelines and practice protocols.
Provide chronic disease management through routine clinics and reviews.
Undertake health screening, immunisations, vaccinations, simple wound care and cervical cytology.
Support and educate patients in lifestyle management, self-care and preventative health measures.
Contribute to infection prevention and control, ensuring safe clinical systems.
Work collaboratively with GP's, other healthcare professionals to ensure continuity of care.
Participate in clinical audits, quality improvements initiatives and the development of practice policies.
Engage in professional development, maintaining registration and compliance with the Nursing & Midwifery Council.
