Main duties of the job

The role involves delivering evidence-based practice for patients with chronic conditions and providing preventative health care to the practice population. The role requires critical thinking and clinical decision-making skills, with flexibility and innovation in working hours and practices.

Person Specification: Minimum requirements

Qualifications and Experience:

• Minimum 1 year of General Practice experience
• Minimum 2 years post-registration experience
• At least 1 year of recent community nursing experience
• Experience managing chronic conditions, implementing protocols, participating in quality initiatives, and audit experience
• Clinical supervision training and experience

Knowledge:

• Management of chronic conditions
• Accountability in primary care roles
• Health promotion strategies
• Local and national health policies
• Health economy and clinical governance issues
• Patient group directions and public health issues

Skills:

• Clinical leadership
• Skills in cervical cytology, immunisation, ear care, minor surgery
