A health organization is seeking a practice nurse to join their team in Hungerford. This role involves managing long-term conditions, supporting patient care, and working within a multidisciplinary team. Ideal candidates will possess qualifications in nursing and experience in chronic disease management. We are looking to recruit a new practice nurse to our clinical team at Lambourn Surgery. You will either be new to practice nursing, looking for your first post, or a more experienced nurse. We would ideally like 2 days a week working Mondays and Fridays, but we can be flexible for the right candidate. You will also cover extended hours on a rotational basis which will be paid as additional hours.
We are a training practice, with regular opportunities for CPD. We offer a minimum of one weeks study leave pro-rata per annum. Additional requests would be on a case by case basis.
The successful candidate will be responsible for theeffective management of patients with long-term conditions using evidence-basedpractice including care for elderly and housebound patients.
The post holder will ensure nursing services are delivered effectively to the patient population.
The Practice Nurse will be responsible for a number of clinicalareas such as infectionprevention and control, health promotion, chronicdisease management, health promotion, well women and well man clinics, as wellas actively supporting the practice management team in the reviewing anddelivery of clinical policy and procedure.
We will ensure that the post holder has access toappropriate clinical supervision and a named mentor to provide general advice and support on a day-to-day basis.

The successful candidate will be working within a multidisciplinary team of nurses, GP's, clinical pharmacist and a skilled administrative support team.
We are looking for a treatment room nurse who can confidently cover all the normal treatment room requirements as listed in the job description. An interest in Diabetes would be an advantage but not essential as training can be given.
Experience of long term condition management especially with diabetes would be an advantage, but not essential as training can be given.
Lambourn Surgery is based 10 minutes from junction 14 and 15 minutes from junction 15 of the M4 making it ideal for a candidate from Swindon or Newbury direction.
We are a small rural practice in the heart of the racehorse community serving 6500 patients across a wide practice area. We have an extended team of Doctors, nurses HCA, Clinical pharmacist and social prescriber.

To support the team in dealing with clinical emergencies
To partake in vaccination programmes for adults and children
To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
To support the clinical team with all safeguarding matters, in accordance with local and national policies
