Main duties of the job
As a Practice Nurse, you will play a key role in delivering high-quality patient care, working in accordance with the NMC Code of Conduct.You will provide assessment, treatment, screening, and health education to our patients, working independently and alongside our team of nurses and GPs.
This role is ideal for someone with experience in Smear testing, chronic disease management, Asthma, Diabetes, wound care, health screening, and risk assessment.
As a Practice Nurse, the successful candidate will be responsible for the safe and effective delivery and future development of the overall service, working towards the achievement of the Practice aims and objectives and supporting the delivery of the Practice business plan.

Job responsibilities
KEY RESPONSIBILITES ANDACCOUNTABILITIES
Management of chronic disease
- Identifying significant abnormalities.
- Diagnosis, monitoring and development of individualmanagement plans; agreeing these as appropriate with the patient and otherhealth professionals.
- Ensuring that all investigations required for QOF orEnhanced services are carried out and coded efficiently in patient records.
- Carry out appropriate tests i.e Spirometry, reversibility.
Therapeutic monitoring
- Checking compliance with and adherence to appropriatetreatments using an holistic patient-centred approach.
- Recognising abnormalities.
- Identifying the impact of treatment and implementing orrecommending changes as appropriate.
Wound Care and management
- Assessing routine wounds including trauma.
- Assessing and prescribing to support good wound managementin complex situations e.g. use of Doppler technique.
- Identifying significant abnormalities.
- Obtaining appropriate information using a lifestylequestioning approach andidentifying appropriate health promotionissues.
- Working with patients to develop a management plan wherehealth problems orpotential health problems are identified.
- Recognising issues and gathering sufficient information torefer e.g. drugs;domestic violence; child protection; vulnerableadults; senior patients; socialproblems.
Health screening
- Recognising issues and gathering sufficient information torefer e.g. womens and mens health, sexual health, older people.
- Undertaking monitoring tasks e.g. smears and providingadvice as appropriate.
- Providing in depth monitoring and advice as appropriate tothe level of the post holders specialty and working with the patient indeciding on management plans.
Travel health
- Administering injections and providing guidance inaccordance with guidelines.
- Identifying problems.
- Provision of specialist and evidence based support to theteam.
Immunisation (adult and child)
- Working with patients with more complex immunisation issues.
Minor operations
- Assisting the general practitioner and/or undertaking somesimple procedures.
- Providing routine ear care.
- Dealing with more complex problems and prescribing ifappropriate.
Mental health
- Communicating with key workers.
- Administering appropriate prescribed therapies andmonitoring for side effects.
- Provide a safe and comfortable environment for the patient.
- Clear awareness on patient consent.

Blood pressure monitoring
- Follow practice policy.
- Awareness of safe and comfortable procedure.
Cardiovascular/NHS Health Checks
- Awareness of National Framework & local objectives.
