Main duties of the job
As a key member of our nursing team, you will be responsible for:
- Delivering comprehensive Chronic Disease Management: This will include managing and reviewing patients with conditions such as Diabetes, Asthma, COPD, Hypertension, and CHD, ensuring adherence to national guidelines and best practice.
- Performing Cervical Cytology: Competently and confidently undertaking smear tests, adhering to national screening programmes and quality standards.
- Supporting patients with complex long-term conditions to self-manage and improve their health outcomes.
- Providing health promotion and preventative care advice.
- Administering immunisations (childhood and adult) and vaccinations.
- Wound care and dressings.
- Undertaking other general practice nursing duties as required.
- Maintaining accurate and up-to-date patient records using SystmOne.

Job Purpose
To act as a practice nurse working in conjunction with other healthcare professionals within a range of primary care facilities to assess and deliver a wide range of primary care nursing duties including venepuncture, wound care, cervical smears, vaccinations, immunisations and chronic disease management.
Key Responsibilities
- To deliver a comprehensive range of primary care nursing interventions in a safe and evidence based method.
- To act autonomously in the provision of nurse led clinic utilising specialist knowledge and experience.
- Opportunistic screening of all patients seen to ensure patients needs are met and recorded appropriately.
- Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills.
- To provide advice around family planning which can include complex and detail regimes.
- Work with nurses and GP to ensure that Quality Outcome frameworks are met (eg Coronary Heart Disease/Older People/Diabetes/Mental Health) and are being delivered.
- Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery.
- Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review.
- To contribute to the clinical development of the practice by participating in service reviews and reflective practice.
- Maintain their professional registration and training needs.
- Record accurate consultation data in patient records.
- Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation.
- Ensure all stock including vaccinations are plentiful and kept in an orderly way and that dates are checked on a monthly basis.

- Essential - Significant experience as a Practice Nurse within primary care.
- Essential - Recognised qualification and demonstrable experience in Chronic Disease Management (e.g., Diabetes, Asthma/COPD Diploma).
- Essential - Current and up-to-date Cervical Cytology training and experience, with demonstrable competency in smear taking.
