Job Summary

We are looking to recruit an enthusiastic and motivated Practice Nurse. Experience of working in General Practice is essential. You must be ready to embrace new projects and provide a high standard of patient care.

Responsibilities

Working in accordance with the NMC Code of Conduct, the post-holder will provide assessment, treatment, screening, health education services and advice to patients. The post-holder will work with reference to practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners. The post-holder will have the required level of training and competence and will work within patient group directions where these are available. A nurse with prescribing qualifications will use them appropriately.

Clinical Duties
  • Management of chronic disease – identify abnormalities, develop management plans.
  • Therapeutic monitoring – check compliance, recognise abnormalities, recommend changes.
  • Patient health checks – obtain information, identify health promotion issues, develop management plans.
  • Risk assessment – recognise issues, gather information, refer appropriately.
  • Health screening – recognise issues, provide advice, undertake monitoring tasks.
  • Travel health – administer injections, provide guidance, identify problems.
  • Immunisation (adult and child) – administer appropriate immunisation autonomously, work with complex issues.
  • First contact – work independently or alongside the GP and make decisions/recommendations.
  • Minor operations – assist the GP and undertake simple procedures.
Qualifications
  • Registered Nurse – NMC registration.
  • General Practice Experience – chronic disease experience, infection control, health and safety, safeguarding knowledge.
  • Experience using EMIS clinical system (desirable).

  • Articulate and knowledgeable to deliver high standard of care.
  • Communicate effectively with patients, relatives, carers and multidisciplinary team.
