A large, forward-thinking healthcare practice is seeking an experienced Practice Nurse to join its multidisciplinary team. The role involves managing chronic diseases, providing vaccinations, and supporting general patient care within a friendly, professional environment serving approximately 25,000 patients. We are a large forward thinking practice looking to recruit an experienced and enthusiastic Practice Nurse to join our growing team of multi-disciplinary clinicians, to deliver high quality care which meets the needs of our patient population whilst working within their professional boundaries.
We are ideally looking for a nurse with experience to provide and maintain a high standard of nursing care basedon the needs of the Practice population which includes treatment, screening,preventative care, chronic disease management and patient education, as well as providing nursingassistance to the Doctors and other members of the primary healthcare team.
We would however be willing to provide training in all areas for the right applicant.
We pride ourselves in offering afriendly, efficient and professional service for approximately 25,000 patientsoperating over 3 sites, two sites are in Gorleston and one in Hopton.
You will work alongside our Practice Nurse Manger and a team of Practice Nurses, HCAs, Phlebotomists, supported by GPs, Advanced Nurse Practitioners, Paramedics, Pharmacists, Pharmacy Technicians, a Managerial team and administrative support staff.

Duties andResponsibilities
Clinical
To manage patients with chronic disease, running specificclinic sessions
Assess, plan, provide, implement, and evaluate nursing careto meet the care needs of individuals in the practice population.
Assess and care for patients with presenting woundsincluding leg ulcers and refer to leg ulcer clinic where appropriate accordingto leg ulcer protocol.
Participate in national screening work such as cervicalcytology and deliver the national immunisation and vaccination schedule
Provide travel health advice and administer appropriatevaccinations
Treat patients with minor injuries
Provide directly and assist GPs with contraceptiveservices
Act as chaperone when required
Undertake health promotion to help educate patients
Select, carry out and interpret results of specific tests
Follow infection control procedures as per infectioncontrol protocol and record as appropriate.
Administration
To maintain accurate patient records and enter onto thecomputer using agreed system templates and read codes
Ensure accurate completion of all necessary documentationassociated with patient health care
To collect data and participate in regular Practice auditand research projects
To contribute to the preparation of any Practicedevelopment plans
General
To maintain own annual registration with the NMC andadhere to Code of Professional Conduct
To maintain a safe and therapeutic environment for all staff, patients, carers and co-workers from other agencies. Ensure allstatutory regulations (e.g. Health & Safety, COSHH, etc) are implementedand adhered to
To promote a professional approach at all times by attireand attitude to patients and staff both in and out of the Practice
To maintain own continuous professional development andensure PDP kept up-to-date in line with the requirements of the Practice
Attend and participate in staff and Practice meetings andin-house clinical training sessions
Any other reasonable duties that may be required from time-to-time
