Job description

An excellent opportunity has arisen for a Practice Nurse to join our friendly team. The post holder will be responsible for the delivery of practice nursing care to our practice population, supporting patients to be healthy, monitoring long-term conditions, and conducting health prevention and screening activities. They will work collaboratively with the general practice team to meet patient needs and support the delivery of policies and procedures.

Job responsibilities

The post holder will be responsible for managing patients with long-term conditions using evidence-based practice, including care for elderly and housebound patients. Working as a senior member of the multidisciplinary team, they will ensure nursing services are delivered effectively. Responsibilities include infection prevention and control, health promotion, chronic disease management, and supporting clinical policy review and implementation. The organization will ensure access to clinical supervision and support from a designated individual.
