Locum Ready is seeking a suitably qualified Practice Nurse to join a GP practice in Ellesmere Port. The role focuses on chronic disease reviews, wound care, immunisations if trained, and routine nursing clinic duties in a structured primary care setting.

You will support high-quality patient care with clinical reviews, lifestyle assessments, recalls, and clear documentation in the EMIS/SystmOne/Vision systems.

Qualifications

• NMC-registered nurse with primary care experience.
• Immunisation training and DBS enhanced.
• Confident in chronic disease management and treatment room-work.
• Good patient communication and clinical reasoning.
• Familiarity with EMIS, SystmOne or Vision.
• Enhanced DBS.

Responsibilities

• Conduct chronic disease reviews for patients with asthma, COPD, diabetes or hypertension.
• Perform wound care, dressings and treatment room duties.
• Carry out adult and children’s immunisations (if trained).
• Undertake smear tests (cytology-trained nurses).
• Complete NHS Health Checks and lifestyle assessments.
• Manage results, recalls and routine follow-up tasks.
• Document clearly in the clinical system.

This role is ideal for a Practice Nurse with strong assessment skills who can confidently deliver chronic disease reviews, treatment room procedures and routine nursing clinics in a structured primary care environment.

You will contribute to high-quality patient care through a balance of clinical reviews, preventative health work and treatment room activity. The practice offers well-organised workflows, supportive nursing leadership and a clear structure for daily clinic delivery.
