A healthcare provider is seeking an experienced Practice Nurse to join their dedicated team in Derby. You will perform essential nursing duties, manage chronic diseases, and have the opportunity to mentor others. We have a fantastic opportunity for an experienced Practice Nurse tojoin our amazing team at our surgeries in Derby.

We are looking forpassionate, patient-centred individuals, that have a demonstrable career ingeneral nursing, with practice nursing experience being preferable although notessential. A background in chronic disease management including asthma anddiabetes is essential for this role, as is the ability to work well both withina team and as an individual, to best serve our large and diverse patientpopulation across our five Derby sites.

You will be expected to perform general practice nursing duties such as, but not limited to, wound care management, health and lifestyle reviews, and vaccinations as part of your role. You will also have the chance to supervise and mentor Healthcare Assistants as part of their professional development, and to take part in shaping the nursing service to address new challenges.

DUTIES & Responsibilities
Assessment, management and treatment of conditions within competence and as delegated.
- Chronic Disease Management such as: Asthma, COPD, Diabetes, Learning Disabilities, Long Term Conditions, NHS Health Checks
- Observations such as: ECGs, blood pressures, bloods, foot checks, urine analysis
- Procedures such as: spirometry, ear syringing, cervical cytology
- Wound care and minor injuries management i.e. removal of sutures and dressings
- General health and lifestyle advice
- Injections such as: B12 injections, hormone injections i.e. Depo
- Vaccinations such as: childhood immunisations, travel vaccinations, shingles, pneumococcal and shingles
- QoF and Enhanced Services
- Referrals
Record Keeping and Administration:
- Accurately label and document patient samples and submit for collection and analysis
- Contribute to nursing team record keeping (clinical, administrative, health and safety related records)
- Maintain skills in using patient appointment system
- Make accurate and contemporaneous records of all patient encounters in accordance with practice records, protocols and coding protocols
Other Duties:
The Practice Nurse may, from time to time, be asked to undertake other reasonable duties, with appropriate training. It is important that all members of staff are prepared to undertake additional or relinquish current duties to maintain service delivery to patients. It is expected that requests from the Nurse Manager / Partners / Operations Manager will not be unreasonably refused.
