We are looking for a Practice Nurse to join our friendly team.
You will provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF.
Hours: 15 per week to full-time.
Main duties of the jobAssess the needs of patients incorporating evidence-based practice
Follow individual treatment plans for chronic disease patients
Identify, manage and support patients at risk of developing long-term conditions
Provide wound care
Support the Infection Prevention Control (IPC) lead in IPC compliance
Provide imms and vaccines
About usWe are a forward thinking and innovative practice with approximately 14,000 patients, based over two sites.
We have 4 GP Partners and a committed team of clinical and non-clinical staff.
Our nursing team consists of 1 Practice Nurse, 2 TNAs and 1 HCA.

Provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
Assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
Identify, manage and support patients at risk of developing long-term conditions
As required, to diagnose and manage acute and chronic conditions
To develop, implement and embed health promotion and wellbeing programmes
Provide wound care (ulcer/Doppler, etc.) to patients
Support the Infection Prevention Control (IPC) lead and to work to the IPC requirements by providing audits, training, support and evidence of compliance
Provide travel medication and vaccines
Request pathology services and process pathology results as required
Maintain accurate clinical records in conjunction with extant legislation
Ensure the appropriate maintenance and use of chronic disease registers
Ensure compliance with the practice chaperone policy
Implement vaccination programmes for adults and children
Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
Deliver opportunistic health promotion where appropriate
