We have an exciting opportunity for a highly motivated and experienced practice Nurse to join our practice team.
You will be joining an excellent forward-thinking PMS practice committed to maintaining the high standards of healthcare. We are situated in Crewe, Cheshire; We have a practice size of 31,000 patients. We consistently achieve high QOF along with other clinical targets, while participating in all local and national enhanced services.
Your skills and experience should include, Childhood and Adult immunisations, Cervical Screening, Chronic Disease Management (diabetes, COPD, asthma, hypertension, A.F and heart failure), general treatment room work (injections).
We are a 7 partner and 5 salaried GP practice based in Crewe, Cheshire. We have a wide range of clinical staff including, Advanced Care Practitioners, ANP's, Paramedics, Triage Nurses, Practice Nurses, Nursing Associates, HCA's and Pharmacists.
We have high QOF achievements each year and consistently acheive our immunisation and vaccination targets.
The following are the core responsibilities of the practice nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
a. To develop, implement and embed health promotion and well-being programmes
b. To implement and evaluate individual treatment plans for chronic disease patients
c. To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health in particular to patients with Diabetes and other long term conditions
d. To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
e. To provide wound care to patients
g. To request pathology services as necessary
h. To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
i. To maintain accurate clinical records in conjunction with extant legislation
j. To ensure read codes are used effectively
k. To maintain chronic disease registers
l. To develop, implement and embed well woman clinics
m. To develop, implement and embed well man clinics
n. To chaperone patients where necessary
o. To prioritise health issues and intervene appropriately
p. To support the team in dealing with clinical emergencies
q. To recognise, assess and refer patients presenting with mental health needs
r. To implement vaccination programmes for adults and children
s. To support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
t. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
u. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
v. To support the clinical team with all safeguarding matters in accordance with local and national policies
w. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
x. To deliver opportunistic health promotion where appropriate
