We have an exciting opportunity to welcome an experienced Practice Nurse to lead clinics that deliver evidence-based practice to patients who are diagnosed with long-term health conditions. Management of diabetes is essential. Knowledge or experience in other areas, such as hypertension, peripheral vascular disease, chronic kidney disease, and cerebrovascular disease would be desirable.

Although the above is the main purpose of this job, the successful candidate will also have the opportunity to provide a general practice nurse service that may include cytology, wound care, immunisation, and other core General Practice Nurse roles as required and within your competencies.
