Job description

We have an exciting opportunity to welcome an experienced Practice Nurse to lead clinics that deliver evidence-based practice to patients diagnosed with long-term health conditions. Management of diabetes is essential. Knowledge or experience in other areas, such as hypertension, peripheral vascular disease, chronic kidney disease, and cerebrovascular disease, would be desirable.

Although the main purpose of this job is as described, the successful candidate will also have the opportunity to provide a general practice nurse service that may include cytology, wound care, immunisation, and other core General Practice Nurse roles as required and within your competencies.

Hours and work pattern: Part-time over two or three days (this is flexible).

Main duties of the job

The focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management and preventative nursing interventions. Management of diabetes is essential, and experience in other areas like hypertension, peripheral vascular disease, chronic kidney disease, and cerebrovascular disease is desirable. The post also involves providing a general practice nurse service including cytology, wound care, immunisation, and other core roles within your scope of practice and competence.

The Porch is a purpose-built practice located in the picturesque market town of Corsham, near the Cotswolds, with easy access to the M4 and 9 miles from Bath. We pride ourselves on providing high-quality, patient-centred care with a strong team ethos.

The practice team includes six GP Partners, three salaried GPs, an ANP, senior and practice nurses, HCAs, phlebotomists, and administrative staff. We benefit from several PCN-employed staff, including physiotherapists, a clinical pharmacy team, and a living well team with social prescribers, care coordinators, physicians' associates, and practice assistants.

Job responsibilities

The core responsibilities include, but are not limited to:

  • Working within the NMC Code of Conduct, maintaining professional standards and behaviour.
  • Recognising and working within your competencies.
  • Diagnosing and managing patients with long-term conditions, integrating drug and non-drug treatments.
  • Implementing and evaluating individual treatment plans.
  • Supporting patient adherence to treatments and reviewing medication effectiveness.
  • Identifying and supporting patients at risk of developing long-term conditions.
  • Providing health promotion and well-being support, including motivational interviewing.
  • Documenting patient reviews and liaising with administrative staff regarding recalls.
  • Providing clinics such as cytology, wound care, ECG, phlebotomy, and administering medications within your scope.
  • Running influenza clinics and providing opportunistic vaccinations.
  • Collecting pathological specimens and requesting pathology services.
  • Recognising mental health needs and acting accordingly.
  • Maintaining cold chain protocols and infection control standards.
  • Prioritising workload and managing time effectively.
  • Supervising students and newly qualified staff as needed.
  • Delegating responsibilities appropriately.
  • Contributing to practice targets and quality initiatives.
  • Engaging in continuous professional development and sharing knowledge with the team.
  • Participating in clinical audits, incident reporting, and protocol development.
  • Attending meetings and building relationships with external organisations.
  • Promoting safeguarding and welfare of children and adults.
