Job description

ThePorch Surgery has an exciting opportunity to welcome an experienced Practice Nurse to lead clinics that deliver evidence-based practice to patients with long-term health conditions. Management of diabetes (or willingness to train) is essential, with support, training, and mentoring provided. Knowledge or experience in other chronic/long-term conditions would be desirable.

The successful candidate will, if they wish, have the opportunity to provide a general practice nurse service that may include cytology, wound care, immunisation, and other core General Practice Nurse roles as required by the surgery and within the successful candidate's competencies.

Hours and work pattern: part-time (16 or 20 hours) over two or three days (flexible and dependent on agreed hours). Support to cover our Monday evening extended access requirement (6.30 to 8 pm) will be part of the working pattern.

Main duties of the job

The role focuses on delivering evidence-based practice for patients with long-term conditions, including management and preventative nursing interventions. Management of diabetes is a key area; other areas such as hypertension, peripheral vascular disease, chronic kidney disease, and cerebrovascular disease are desirable. The post may also include providing a general practice nurse service, including cytology, wound care, immunisation, and other core roles within your competencies.

The Porch is a purpose-built practice located in Corsham, near the Cotswolds, with easy access to the M4 and 9 miles from Bath. We pride ourselves on high-quality, patient-centred care with a strong team ethos.

Our team includes six GP Partners, three salaried GPs, an ANP, senior and practice nurses, HCAs, phlebotomists, and administrative staff. We benefit from several PCN-employed staff, including physiotherapists, a clinical pharmacy team, and a living well team with social prescribers and care coordinators.

Job responsibilities

The core responsibilities include:

  • Working within the NMC Code of Conduct, maintaining professional standards.
  • Recognising and working within own competencies.
  • Diagnosing and managing patients with long-term conditions, integrating drug and non-drug treatments.
  • Implementing and evaluating individual treatment plans.
  • Supporting patient compliance and adherence to treatments.
  • Reviewing medication effectiveness and discussing changes with GPs if necessary.
  • Identifying and supporting at-risk patients to prevent adverse effects.
  • Providing health promotion and wellbeing support, using motivational interviewing.
  • Accurately documenting recalls and liaising with administration.
  • Providing clinics such as cytology, wound management, ECG, phlebotomy, and administering medications within scope.
  • Running influenza clinics and administering vaccines as per PGDs.
  • Checking immunisation status and offering vaccines.
  • Chaperoning patients and collecting specimens.
  • Requesting pathology services and recognising mental health needs.
  • Monitoring cold chain and infection control standards.
  • Prioritising workload and acting as a supervisor when needed.
  • Contributing to practice targets and maintaining professional development.
  • Participating in clinical supervision, audits, and meetings.
  • Ensuring safe medication storage and developing relationships with external organisations.
  • Participating in initiatives to improve service delivery and safeguarding.
