We are looking for a motivated and caring experienced Practice Nurse to join our friendly and supportive Practice team. We are a progressive and forward-thinking Practice who are passionate about providing high quality patient care.

Main duties of the job
  • Working as part of the Practice Nursing Team
  • Providing assessments, screening, treatment services, and health education advice
  • Manage leg clinic and facilitate leg management pathway
  • Prepare/assist/facilitate vaccinations and immunisations
  • Undertake collection of pathological specimens
  • Provide appropriate follow ups for patients
  • Lead in an area of chronic disease management
  • Ensure accurate notes are recorded
  • Attend and participate in practice meetings as required
  • Assist in formulation of strategy and policy
  • Undertake administrative procedures
  • Participate in collection of stats for audits
  • Opportunity to be involved with our COVID Medicine Delivery Unit providing infusions

ClinicalResponsibilities

  • Working as part ofthe practice nursing team
  • Provide Assessment,Screening, Treatment Services and Health Education advice
  • Provide nursingtreatments to patients in participation with general practitioners orindependently to agreed protocols
  • Provide general andspecific health screening to the practice patients (within agreed protocols)with referral to the general practitioners as necessary
  • Give appropriatehealth promotion advice to patients in whom risk factors are identified,according to agreed practice protocols and local and national targets
  • Advise patients ongeneral health care and minor ailments with referral to general practitionersas necessary
  • Participate inhealth promotion for patients with chronic diseases such as diabetes, asthmaand CHD, according to agreed practice protocols.
  • Manage leg clinicand facilitate leg management pathway.
  • Prepare/assist/facilitatevaccinations and immunisations and travel programmes
  • Undertake cytologyscreening or be willing to learn.
  • Undertake thecollection of pathological specimens including intravenous blood samples,swabs, etc., and perform any investigatory procedures requested by the GPsfollowing appropriate training
  • Provide appropriateregular follow up for patients when necessary
  • Take the lead in anarea of chronic disease management

Administrative andProfessional Responsibilities

  • Participate in theadministrative and professional responsibilities of the practice team
  • Ensure accuratenotes of all consultations and treatments are recorded in the patientscomputerised medical records
  • Ensure accuratecompletion of necessary documentation associated with patient health care andregistration with the practice
  • Attend andparticipate in all practice meetings as required
  • Assist in theformulation of practice philosophy, strategy and policy
  • Undertake alladministrative procedures relating to the post, liaising with the Nurse TeamLead
  • Co-operate andparticipate in the collection of statistics for audit projects and evaluate andpresent results as required
  • Participate in thereview of existing evidence-based nursing practice by using valid and reliableaudit tools and published and validated research results from medical, nursingand paramedical data to effect changes in clinical practice
  • Cover in theabsence of other treatment room staff as and when necessary

Infusion Clinics

  • Have theopportunity to be involved with our COVID medicine delivery unit providinginfusions for our vulnerable population requiring treatment.

Education andTraining of Students and Practice Staff

  • Participate in theinduction of all members of practice staff. To be available for students of alldisciplines to observe clinics, answer questions arising from these clinics andteaching new skills where necessary
  • Maintain effectiveliaison with other agencies and staff concerned with patient care and with allother disciplines

ProfessionalDevelopment

  • Maintain continuededucation by attendance at course and study days as deemed useful or necessaryfor professional development and to maintain PREP requirements
