A local healthcare provider in Rossendale is seeking a part-time practice nurse to join their team. Responsibilities include managing chronic diseases, performing cytology testing, and running vaccination clinics. Ideal candidates should be registered nurses with experience in chronic disease management and excellent communication skills. We are looking for an experienced and motivated part time practice nurse to join the team here at St James Medical Centre 27.5 hours per week over 3 days. We are based in leafy Rawtenstall in the valley of Rossendale close to good motorway links.
The successful candidate will be part of a team providing practice nursing services to our patient population. Main duties include;
We have a population of 8500 patients. We are a friendly, organised, forward thinking practice. We have the benefit of a number of ARRS staff clinics such as PA, TNA, Mental Health Practitioner, First Contact Physio and Health and Wellbeing coach. We are a training practice and this fosters a culture of support for our staff. We have excellent clinical and administration teams and effective processes in place to help clinics run smoothly.
Assess, plan, provide and implement and evaluate nursing care to meet the care needs of individuals in the practice population
Provide access to specialist nurse/GPs
Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
Check all emergency equipment and drugs regularly and ensure records are up to date.
Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care e.g. diet, exercise, alcohol, smoking cessation
Actively maintain health promotion material within the practice and waiting room notice boards
Undertake health screening, surveillance, new-patient screens and immunisation
Maintain patient confidentiality and accurate record keeping
Attend and participate in practice team and clinical meetings
Vaccination and immunisations, administering injections, ensuring safe vaccine storage, implementing cold chain policy.
Mentor and assess trainees and junior team members.Specific Health Care responsibilities:
Child Care and Child Protection
Recognise signs, symptoms and categories of child abuse.
Understand importance of recognition, observation, documentation and communication.
Is aware of child health procedures, statutory local procedures and points of referral.
Has knowledge of other practitioners and agencies roles.
Understands basic legal issues around child abuse.
