The NHS is seeking an experienced Practice Nurse to join a vibrant teaching practice. The role involves managing chronic conditions, implementing health promotion programmes, and providing holistic patient care. Candidates need to be registered nurses with relevant qualifications and experience.

We have a great opportunity for an enthusiastic Nurse to joinour large, vibrant, and friendly teaching Practice. We arelooking for the right person to join our Nursing team that includes aNurse Manager, Practice Nurses, HCAs and Phlebotomists.
We pride ourselves on delivering high quality holistic care to ourpatient population and have a culture in learning and teaching in all we do. We are looking for a forward thinking Nurse.
The successful applicant will have the experience and qualifications to advise patients in health prevention, carrying out screening and early management of long term health conditions.

We are a large training practice with teams comprising of clinical and non clinical staff, with the experience and knowledge to support the Practice. We have weekly clinical meetings, regular supervision sessions for our staff who are still learning/training.

Develop,implement and embed health promotion and well-being programmes
Manage patientspresenting with a range of acute and chronic medical conditions, providingsubject matter expert advice to patients
Implement andevaluate individual specialised treatment plans for chronic disease patients
Identify,manage and support patients as risk of developing long-term conditions,preventing adverse effects on the patients health
Provide woundcare (ulcer /Doppler etc.) to patients
Providespecialist clinics such as contraception
Undertake the collectionof pathological specimens
Requestpathology services as necessary
Process andinterpret pathology and other test results as required
Provide chronicdisease clinics, delivering patient care as necessary, referring patients tosecondary / specialist care as required
