Rush Hill and Weston Surgery are looking for a passionateand motivated nurse to join our dynamic practice nursing team in a busy GPsurgery setting. This is an exciting opportunity for a nurse who is driven todeliver high standards of care and is eager to develop their skills within asupportive primary care environment.
Previous experience of primary care nursing is desired butnot essential if you can demonstrate excellent transferable skills and apassion for practice nursing.

REPORTS TO:Lead Practice Nurse
ACCOUNTABLE TO:GP Partners (Clinical)&Practice manager (Organisational and Administrative)

You will work collaboratively with our multidisciplinaryteam, offering support, guidance and education to others, while also developingyour ability to work autonomously. Full training and support will be providedto help you grow into the role and take on new responsibilities confidently.
The role will involve managing a diverse clinical case load,delivering personalised care, supporting patients in the management oflong-term conditions, and promoting health education and prevention. You willplay a key part in encouraging patients to take control of their health throughevidence-based and compassionate care.
(Furthermore, there is an exciting opportunity to workalongside the current lead nurse to develop leadership and team managementskills with the prospect of an established leadership role within the team).
The post holder will be expected to contribute to audit, education,research and supporting, implementing and evaluating local standards, policies,guidelines and care pathways. NMC requirements will be met, and the individualwill work within the NMC Code of Conduct to agreed practice standards andprotocols.
Rush Hill Surgery is situated on the Southwest hills of the city of Bath with a branch site at Weston surgery in the Northwest of Bath. We cover a large area and have 9100 patients registered to our practice. We are part of the Minerva Primary Care Network and work closely with our partners to meet the needs of our patients.
We offer high quality primary care health services for the area that is responsive to our population needs. Our dedicated nurse team treats minor ailments as well as providing specialist management of long-term conditions and wider healthcare issues and offer extensive health promotion.
MAIN DUTIES & RESPONSIBILITIES OF POST HOLDER
Meeting patients nursing care needs asidentified, or where care has been transferred to them by a GP. This will usually be in surgery, butoccasional home visits may also be required.
Initiating and maintaining health screening andpromotion clinics for identified areas of the practice population, recognisingwhere modification of systems or referral is necessary.
Assess, plan, develop, implement and evaluatetreatment programmes for patients whilst promoting health and well-being.
Employ a shared care approach to supportingpatients managing long term conditions and chronic diseases. This includes butis not limited to, requesting diagnostic metrics, operate diagnostic equipment,ability to interpret clinical information and provide explanations to patientsin a sensitive manner, undertake regular medication reviews within your scopeof practice and arrange follow up review suitable to patients and practicesneeds, using the recall systems.

-Cardiovascular disease
-Cervical screening
-Family planning and sexual health
-Learning Disability Health Assessments
-Wound management (simple and complex wounds)including doppler assessment and compression bandaging
-Suture/clip removal
-Venepuncture
-Childhood vaccinations (routine and unscheduled)
-Travel immunisations and advice
-ECGs
-General health screening including height,weight, BP, urinalysis, smoking status, alcohol and lifestyle.
-Knowledge of and ability to explain rationalebehind national and local screening programs and promote uptake ofscreening. Access appropriate resourcesto promote health literacy and screening information.
-Use of motivational techniques to supportlifestyle modifications and behaviour change.
-Effectively communicate with and refer tooutside agencies to support all aspects of health management ensuringcontinuity of care.
Apply infection control measures within thepractice according to local and national guidelines.
Support patients and colleagues to adopt evidence-basedInfection and Prevention Control (IP&C) measures to prevent the developmentof health care associated infections in line with the practices health andsafety policy and antimicrobial strategy to reduce antimicrobial resistance.This will include:
-Effective hand hygiene following the 5 momentsfor hand hygiene.
-Applying universal precautions
-Appropriate and safe management of specimensincluding disposal
-Appropriate waste material segregation anddisposal
-Effectively and safely dealing with blood andbodily fluid spillages
-Reporting and management of sharps injuries
To be able to recognise a deteriorating patientand appropriately escalate concern to senior colleagues.
To be competent in managing emergency situationsincluding completion of annual basic life support (BLS) and resuscitationtechniques and anaphylaxis.
Maintain contemporaneous and accurate patientrecords.
Use the practice computer systems for input ofinformation for recall and screening purposes.
Contribute to clinical audit and the setting upand monitoring of standards of care.
Assist in education of others within theclinical setting, including unqualified staff.
Act as assessor and mentor to other staff wherenecessary including student health care professionals.
Work effectively in the team demonstratingexcellent communication skills.
Maintain your own continuous professionalregistration in line with regulatory body requirements and Practicerequirements.
Be available to deal with any patient query thatmay prove to beyond the ability of the medical receptionist and be aware oflimit of own ability.
Ensure that all formal complaints are passedonto the Practice Manager for immediate action.
Have good IT literacy and use the computer andcommunication systems effectively and professionally.

PROFESSIONAL REGISTRATION
You will be expected to maintainyour own annual registration with the NMC.
