The Practice offers a variety of appointmentsand clinics including Sexual Health, minor injuries, minor surgery, jointinjections, childhood immunisations, spirometry, long term condition monitoring,as well as general Nursing and clinician appointments. Special interests andpersonal development are welcomed and encouraged.
We would prefer a nurse with respiratory qualifications, or who is willing to train in this area.
Our team includes 4 Practice Nurses, 2 HCAs, 4 Partners (3 GPs 1 ANP), 3salaried GPs,1 GP Retainers, a first contact Physiotherapist and a Pharmacist. We have fantastic administrative support and a dynamic management team.We use Emis web, are high QOF achievers and were rated good by CQC in 2019.
To be responsible for the implementation of processes for theeffective management of patients with long-term conditions using evidence-basedpractice including care for elderly and housebound patients.
Working as a member of the practice multidisciplinary team,the post holder will ensure nursing services are delivered effectively to theentitled patient population.
The Practice Nurse will be responsible for a number of clinicalareas such as infectionprevention and control, health promotion, chronicdisease management, health promotion, well women and well man clinics, as wellas actively supporting the practice management team in the reviewing anddelivery of clinical policy and procedure.
This organisation must ensure that the post holder hasaccess to appropriate clinical supervision and an appropriate named individualin the organisation to provide general advice and support on a day-to-daybasis.
