Practice Educator - Community - Band 7

We are transforming Adult Community Services in Croydon. Do you have the skills to be part of this exciting transformation agenda?

In Croydon, Health and Social care providers and commissioners have formed the Croydon Alliance Board with the aim of providing a whole system transformation that will deliver the outcomes our over 65's have specified that they want.

The Outcome-Based Commissioning programme is a radically different approach to funding and delivery of services designed to deliver outcomes local people want whilst getting best value from the Health and Social care sectors. This includes the provision of well-coordinated care and support to promote independence in the most appropriate setting.

We require a part-time Practice Educator to work within our Integrated Care Teams to support staff in developing the advanced skills required to provide effective nursing care at home to avoid hospital admissions. Excellent clinical, leadership, and communication skills are essential along with the ability to work innovatively to support the transition to new Integrated Community Networks. The successful candidate will be passionate about education, quality, and safety, acting as an excellent role model.

All candidates must have a clean full UK driving licence and the use of a car for work purposes.

Main duties of the job

  1. Coordinate and facilitate educational activities for registered and non-registered nursing and midwifery staff within a designated practice area.
  2. Establish and maintain links with Kingston Faculty of Healthcare Sciences, Kings College London, and South Bank University.
  3. Improve patient experience and care standards on the ward/clinical areas.
  4. Support induction content and processes for all newly appointed nursing staff.
  5. Lead and develop competency assessment documents to enhance care standards across the Trust.
  6. Participate actively in national and corporate projects.
  7. Provide pastoral support for nursing staff.
  8. Identify and coordinate CPD activities for nursing staff in collaboration with senior nurse leaders and the Head of Nursing for Practice Development & Education.
  9. Contribute to the development of a trust strategy and career pathways for nursing staff in collaboration with stakeholders.
  10. Lead and develop study days for nursing staff aligned with organisational priorities.
