A leading practice in South Manchester is seeking a Clinical Pharmacist to enhance patient care and operational efficiencies. The role involves collaboration within a dynamic team, providing clinical leadership, and improving the quality of healthcare services. We have a vacancy for a Clinical Pharmacist at Benchill Medical Practice in Wythenshawe, South Manchester.
The hours are 30 per week.
The practice population is diverse and varies in demographics so the breadth of health care need is wide. This makes working in general practice interesting and no day is the same.
We are looking for a Clinical Pharmacist to work as part of dynamic and forward-thinking practice team. You will also be part of a wider supportive network of Senior Pharmacists, Pharmacists and Technicians across the South Manchester Primary Care Networks (PCNs).
You will be embedded within the practice team providing clinical leadership on medicines related issues, quality improvement and supporting aspects of the national and practice based enhanced services, quality and outcomes framework and local targets.
The role is pivotal to improving quality of care and operational efficiencies in the practices so you will need to be motivated and passionate with the drive to continually improve and learn, to develop and deliver an excellent service.
You will be working along side another Clinical Pharmacist, supporting colleagues with with medicine related queries and tasks.

Provide clinical input on medicines related issues, qualityimprovement, supporting aspects of the quality and outcomes framework and locallycommissioned services.
Support repeat/acute prescription requests, medicinesreconciliation on transfer of care and systems for safer prescribing.
Provide face to face patient clinics.
Ensure patients are referred to the appropriate health careprofessional for the appropriate care linking in with social prescribing modelsto support broader health and well-being needs.
Support integration with community and hospital pharmacy tohelp utilise skill mix, improve patient outcomes, ensure better access to healthcareand help manage the practice workload.
The specific tasks will be agreed and identified by the practice. You will be part of the practiceteam but also provided with peer support and education via the South Manchester Pharmacists and Technicians across 22 practices.
