A healthcare provider in Loughborough seeks a Clinical Pharmacist who will manage medication systems, provide consultations, and implement safe prescribing procedures. The ideal candidate will hold GPhC registration and relevant certifications, and be dedicated to improving patient care within a collaborative environment.
Asa Clinical Pharmacist and Independent Prescriber, you will play a key role insupporting safe, effective, and personalised medication management.
You will be responsible for implementing an effective medicine management system withinthe practice. You will be expected to work to achieve cost-effectiveprescribing, work as an independent prescriber, provide advice and support forboth patients and colleagues, undertake all duties expected within the changingrole over time. Lead and take part in audits and quality prescribinginitiatives. Take part in training of others when required. Undertake anyduties as identified within the GP DES, or other contracts. Maintain ownprofessional accreditation requirements. Work will be based in Practice.
Thefollowing are the core responsibilities of the clinical pharmacist. There maybe on occasion, a requirement to carry out other tasks; this will be dependentupon factors such as workload and staffing levels:
a.Practiceas an independent prescriber working within scope of practice (when qualified)
b.Toprovide medication review services with patients by telephone calls, clinicalrecords reviews, face to face reviews or during domiciliary visits
c.Toreconcile medicines following patients discharge from hospital or clinic appointments.
d.Reviewall safety alerts ensuring the required actions are taken.
e.Carryout targeted medication reviews for high-risk patients (e.g., by using eclipse)
f.Reviewand implement safe prescribing systems for vulnerable patients, improving compliance.
g.Toprovide subject matter expertise on medication monitoring, implementing, andembedding a system
i.Completework as specified within the GP DES or other contracts.
Task to be sent to accountable GP if anyalterations have been made to the repeat template if not an independentprescriber as per agreed practice policy

Secondary responsibilities
Inaddition to the primary responsibilities, the clinical pharmacist may berequested to:
a.Toprovide advice and support for patients with self-limiting conditions
b.Encouragecost-effective prescribing within Practice.
c.Reviewmedications for newly registered patients
d.Implementand embed a robust repeat prescribing system.
e.Toreauthorise medication for repeat prescribing within scope of practice
f.Identifypatients suitable for repeat prescribing, liaising with the relevant cliniciansas required.
g.Supportclinicians with the management of patients suffering from drug and alcohol dependencies.
h.Activelysignpost patients to the correct healthcare professional
i.Assistwith QOF targets.
j.Administerinfluenza and travel vaccines.
k.Provideeducation on medicines for the clinical team.
l.Reviewthe latest guidance ensuring The Practice conforms to NICE, CQC etc.
m.Actas the practices point of contact for all medicines related matters,establishing positive working relationships.
n.Producepharmacy / prescribing newsletters or bulletins on a quarterly basis.

Induction Training
On arrival at The Practiceall personnel are to complete an induction programme.
Learning and Development
Theeffective use of training and development is fundamental in ensuring that allstaff are equipped with the appropriate skills, knowledge, attitude and competencesto perform their role. All staff will be required to partake and completemandatory training as directed by the training coordinator, as well asparticipating in The Practice training programme. Staff will also be permitted (subject toapproval) to undertake external training courses which will enhance theirknowledge and skills, progress their career and ultimately, enable them toimprove processes and service delivery.
Allstaff are to recognise the significance of collaborative working. Teamwork isessential in multidisciplinary environments. Effective communication isessential and all staff must ensure they communicate in a manner which enablesthe sharing of information in an appropriate manner.
Service Delivery
Allstaff must adhere to the information contained within the Practice andindividual practice policies and regional directives, ensuring protocols areadhered to at all times. Staff will be given detailed information during theinduction process regarding policy and procedures.
Security
The security of The Practice is theresponsibility of all personnel. Staff must ensure they remain vigilant at alltimes and report any suspicious activity immediately to their line manager. Underno circumstances are staff to share the codes for the door locks to anyone andare to ensure that restricted areas remain effectively secured.
Professional Conduct
Atwork staff are required to dressappropriately for their role. Administrative staff will be provided with auniform whilst clinical staff must dress in accordance with their role.
Leave
Allpersonnel are entitled to take leave. Line managers are to ensure all of theirstaff are afforded the opportunity to take a minimum of 27 days leave each year(pro rata plus declared bank holiday days pro rata), and should be encouragedto take all of their leave entitlement.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
