Job description

We are seeking aproactive and clinically confident Post-registration Pharmacist to joinour friendly and supportive general practice team at The Pontypool MedicalCentre. This is an excellent opportunity for a recently qualified orearly-career pharmacist (up to 24 months post registration) looking to buildtheir clinical skills in a primary care setting.

You will workclosely with GPs, nurses, and other healthcare professionals to ensure safe,effective, and person-centred use of medicines for our patient population.

About us

Pontypool Medical Centre is a multi-site general practice operatingacross several locations.

Our main site is a large, well-equipped surgery located in the centre ofPontypool. We also have two smaller branch surgeries: one in New Inn and adispensing practice in the village of Goytre. In addition, our wider groupincludes practices based in Newport and Gelligaer, offering the opportunity togain experience across a range of different primary care settings.

As a large and dynamic practice with a diversemultidisciplinary team, we are well placed to provide a broad and comprehensivetraining experience for foundation pharmacists

Job responsibilities

Undertakestructured medication reviews and support medicines optimisation

Supportpatients with long-term conditions such as asthma, COPD, hypertension, anddiabetes

Provideprescribing support under clinical supervision (if independent prescribingqualified)

Contributeto safe repeat prescribing systems

Helpmanage clinical correspondence related to medicines

Supportaudit, formulary work, and medicines safety initiatives

Assist inimproving antimicrobial stewardship and reducing medication waste

Person Specification
Qualifications
  • GPhC registration as a pharmacist
  • Completion of pre-registration foundation training
  • Strong clinical communication and interpersonal skills
  • Ability to work both independently and as part of a multidisciplinary team
  • Enthusiasm for working in general practice and delivering person-centred care
  • Familiarity with prescribing systems (e.g. EMIS, Vision)
