Job description
Overview

PPT provide evidence based long-term intervention that supports the development and maintenance of capable environments that reduce the risk of placement breakdown/admission to hospital and enhance the quality of life of service users with complex and challenging needs.

Promoting a Positive Behavioural Support approach PPT provide competency-based training and skills development in; Understanding and Responding to Autism (SPELL Framework), Person Centred Active Support and Understanding and Responding to Behaviours that Challenge.

Established in 2019 the team works with service users, their families and community-based support networks including social workers and CLDHT colleagues across Buckinghamshire. Taking internal referrals from clinicians within the Bucks Community Learning Disability Team, PPT are an integral part of the Bucks pathway supporting people who exhibit behaviour that challenges.

Role
  • Supporting people with a learning disability and challenging behaviour to live in welcoming and supportive communities.
  • Providing personalised, flexible and responsive support to assist individuals, families, services and communities to access accurate and timely information to clarify their goals, strengths and needs.
  • Supporting the delivery of PPT’s training suite and providing opportunities to embed the learning into the day to day.
  • Operating as a service coordinator (rather than a service provider) and helping the referred person and their families/carers to plan, select and receive needed supports and services.
  • Assisting people and their supports/services to manage barriers and constraints in achieving their preferred lifestyle, including:
    • Assisting local services to act early before the barriers and constraints jeopardise the person’s ability to remain in their local community.
