Role and Responsibilities
  • Manage a defined caseload with responsibility for continuity of care and communication to identified stakeholders.
  • Provide proactive, responsive services addressing anticipatory and maintenance care needs through holistic assessment and clinical care for patients/clients with acute and chronic highly complex needs.
  • Practice autonomously, demonstrate evidence-based clinical decision making, identify and manage risk, and report risks to the team leader.
  • Ensure delivery of the quality strategy within the designated area by establishing systems and processes.
  • Provide high-quality care to a defined caseload and participate in workload planning and delegation to team members based on competence levels.
  • Undertake holistic assessments of need for patients and families, possibly highly complex, using appropriate care models and evidence-based practice.
  • Work with other professionals to enable patients/clients to be maintained in the appropriate environment, avoiding hospital admission where possible.
  • Advise patients, clients, and carers on health promotion, lifestyle, and care issues; ensure a high-quality clinical service through caseload and clinical supervision.
  • Collaborate with the team leader to develop new ways of working to meet planned and unplanned care needs; establish partnerships with other agencies as appropriate.
  • Maintain professional registration and provide clinical leadership, mentoring, and support to the team; assist in caseload management across the local health economy.
  • Act as advocate and role model, contribute to policy and service development, and provide information on staff and patient activity for effective management.
  • Support practice development, quality improvement, and clinical governance initiatives; participate in supervision, audits, research as required.
  • Support induction, personal development planning, and learning within the team; supervise and mentor students and junior staff.
  • Ensure compliance with organisation policies and provide cover for absent colleagues as necessary.
Requirements
  • HCPC registered Band 6 Podiatrist with post-registration experience; able to assess and treatment plan for patients with complex needs, including wound management.
  • Ability to work in clinic settings and patients’ homes; strong diagnostic, communication, and clinical skills for a broad range of long-term conditions.
  • Evidence of effective teamwork, professional development, and commitment to patient-centred care.
  • Demonstrable ability to work within multidisciplinary teams and to participate in clinical audits and research as required.
