Job Description: Urgent Care Practitioner
We are seeking a dedicated Urgent Care Practitioner to work across various domains including the Emergency Department, Acute Medical Unit, Acute Frailty Unit, and Same Day Emergency Care (SDEC).
- Collaborate with the multidisciplinary team to promote an efficient, effective service for patients and carers.
- Work as an autonomous practitioner, professionally and legally accountable for all aspects of clinical work.
- Provide therapeutic interventions for symptoms and difficulties related to presenting diagnoses, guided by up-to-date NICE guidelines.
- Manage a designated caseload, assessing patients, and developing and evaluating treatment plans.
- Assist in organizing, developing, promoting, and delivering the service.
- Liaise with health, education, and social services, working in partnership with primary and secondary care, social prescribers, and external agencies to ensure high-quality service delivery.
- Make recommendations for practice changes, lead implementation, and contribute to service guidelines.
- Perform advanced holistic triage and assessment of patients with complex physical, emotional, and psychological health presentations.
- Develop and deliver individualized treatment programs.
- Collaborate with primary and secondary healthcare, third sector, and social services to ensure seamless and high-quality care.
- Work independently with indirect supervision from the clinical lead, supported by regular training, peer review, and MDT support.
- Support the MDT and cover for colleagues as needed.
- Undertake all clinical duties as an autonomous practitioner.
- Advocate for patients and their families when necessary.
- Conduct risk assessments in line with Trust/National protocols.
- Develop patient-centered intervention and management plans proactively.
- Ensure care and interventions are evidence-based and adhere to professional standards and NICE guidance.
- Maintain up-to-date professional standards and literature awareness.
