Job Description: Urgent Care Practitioner

We are seeking a dedicated Urgent Care Practitioner to work across various domains including the Emergency Department, Acute Medical Unit, Acute Frailty Unit, and Same Day Emergency Care (SDEC).

  1. Collaborate with the multidisciplinary team to promote an efficient, effective service for patients and carers.
  2. Work as an autonomous practitioner, professionally and legally accountable for all aspects of clinical work.
  3. Provide therapeutic interventions for symptoms and difficulties related to presenting diagnoses, guided by up-to-date NICE guidelines.
  4. Manage a designated caseload, assessing patients, and developing and evaluating treatment plans.
  5. Assist in organizing, developing, promoting, and delivering the service.
  6. Liaise with health, education, and social services, working in partnership with primary and secondary care, social prescribers, and external agencies to ensure high-quality service delivery.
  7. Make recommendations for practice changes, lead implementation, and contribute to service guidelines.
  8. Perform advanced holistic triage and assessment of patients with complex physical, emotional, and psychological health presentations.
  9. Develop and deliver individualized treatment programs.
  10. Collaborate with primary and secondary healthcare, third sector, and social services to ensure seamless and high-quality care.
  11. Work independently with indirect supervision from the clinical lead, supported by regular training, peer review, and MDT support.
  12. Support the MDT and cover for colleagues as needed.
  13. Undertake all clinical duties as an autonomous practitioner.
  14. Advocate for patients and their families when necessary.
  15. Conduct risk assessments in line with Trust/National protocols.
  16. Develop patient-centered intervention and management plans proactively.
  17. Ensure care and interventions are evidence-based and adhere to professional standards and NICE guidance.
  18. Maintain up-to-date professional standards and literature awareness.
