A leading healthcare provider is seeking a competent Physiotherapist for the Pulmonary Rehabilitation Service in London. This role involves supporting patients with COPD and Home Oxygen therapy, conducting assessments, and working within a multidisciplinary team to enhance patient self-management. You will be expected to be a competent physiotherapist able to adapt to clinical pathways in the COPD and Home Oxygen Service with appropriate training and support. Your main duties will be within the Pulmonary Rehabilitation Service, supporting the PR Lead to provide a high-quality program and work towards accreditation. You will be part of a community team, making high-level clinical decisions to aid patient self-management, liaising with patients' medical and therapy teams, making referrals, and liaising with GPs or other healthcare professionals to promote independence and prevent exacerbations and hospital admissions.
You will provide assessments, interventions, and goal-based rehabilitation, working autonomously and as part of a team to deliver comprehensive self-management plans, including coordinating interventions, signposting patients, and taking a keyworker role. You will use evidence-based principles to guide practice and ensure team members complete delegated work. Your caseload management and communication skills will enable prioritization of patient care. You will collaborate with multidisciplinary teams including doctors, nurses, social workers, rehabilitation assistants, and others to support patient needs.
You will be responsible for your CPD and adhere to professional standards and guidelines. Your clinical duties include conducting specialist assessments, setting treatment goals, maintaining high standards of evidence-based practice, triaging referrals, monitoring interventions, and managing clinical risk. You will also educate carers and health workers involved in rehabilitation care programs and contribute to developing new clinical services.
In terms of education, you will maintain and update your CPD portfolio, participate in training and health promotion initiatives, and comply with mandatory training requirements. Management duties include maintaining accurate records, participating in audits, supporting team members, ensuring equipment safety, and participating in meetings and service evaluations. You will also be involved in research activities and keeping abreast of clinical developments in rehabilitation.
