Job description
Physiotherapist Or Occupational Therapist

The post holder will be required to participate in sickness cover, vacancy cover and weekend service cover across the Emergency Department, Acute Medical and/or Specialist or Elderly Medicine wards, across Charing Cross and St Marys Hospital, depending on service needs. Staff will need to provide thorough holistic assessment and personalised treatment to maximise patients functional ability to facilitate discharges as well as the prevention of inappropriate admissions and readmissions at the Front Door of the Hospital.

Main duties of the job

Identify referrals appropriate for weekend therapy, prioritising and allocating per team procedures, seeking colleague advice as needed.Attend board round meetings as required.Select and apply therapeutic assessments for a designated caseload to evaluate physical, cognitive, psychological, and social functioning, using specialist tools where appropriate.Consider comprehensive geriatric assessments, including cognitive screening tools such as 4AT, AMTS, RUDAS.Plan and implement individual interventions with patients using graded activity to meet therapeutic goals.Monitor, evaluate, and adjust treatment for clients with complex needs to ensure progress and effectiveness.Agree goal-oriented programmes with clients, families, and professionals to support appropriate intervention.Develop discharge plans recommending suitable packages for patients with acute and chronic needs, ensuring timely onward referrals.Consider referrals to step-down community and home-based therapy services to support discharge and therapeutic goals.Liaise with and refer clients to other services per procedures and time constraints.Recommend equipment and/or home adaptations to suit client needs, following relevant policies.Identify complex cases unsuitable for weekend assessment, discharge, e.g. safeguarding concerns, mental capacity issues, or need for MDT input.

Job responsibilities

To provide high quality PT - OT management to patients at Imperial College Healthcare NHS Trust.

To participate in, providing a high standard of care to medically optimised (or soon approaching) patients within the Emergency Department, Acute Medicine, Specialist Medicine or Elderly Medicine wards as priority caseload dictates.

To perform advanced physiotherapy or occupational therapy assessment of patients with diverse presentations and complex physical and psychological and cognitive conditions, to develop and deliver individualised treatment programmes and discharge plans where required.

Deliver effective communication with patients, carers, family, colleagues, and external agencies.

To be responsible for own caseload and be responsible within a defined area of the service and to undertake all aspects of clinical duties as an autonomous practitioner

Maintain accurate patient records and activity data in line with Trust standards.

Uphold professional ethics and respect diversity in all aspects of care.

Manage time and resources effectively, ensuring reliable attendance and adherence to policies.

Person Specification
Qualifications
  • Degree in physiotherapy or occupational therapy
  • HCPC registration or equivalent professional registration as a physiotherapist or occupational therapist.
Experience
  • At least 12 months NHS Experience
  • Bank banding capped at Band 6 however, experienced B5s are welcome to apply and bank grade at managers discretion.
