An exciting opportunity has arisen to join the communityfalls team. This role is for a specialist physiotherapist/occupationaltherapist with skills in assessing and treating patients who are at a High riskof Falls (patients who have had 2 or more Falls within the last 12 months). Youwill be carrying out an comprehensive assessment looking into the risk factorsfor Falls and analysing how to reduce the patients future risk of falls. Thesesassessments may be carried out in a Clinic setting or in a patients own home.Therapy can also be provided in the patients own home or may involve somegroup sessions. As a service we focus on Falls prevention and triage patientswho have visited A&E as a consequence of a fall as well as promotingexercise and education to both patients and carers.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

To be an autonomous practitioner with the freedom to act tobe personally, professionally and legally accountable for the services youprovide to patients in your care and for those Associate Practitioners, Careand Therapy Support workers andTo perform advanced therapeutic assessment of patients withdiverse presentations and sometimes highly complex physical conditions; toprovide a diagnosis, develop, and deliver individualised treatment programme
To hold responsibility for own caseload and be responsiblefor a defined area of the service or a particular patient type, working withoutdirect supervision.
To be operationally managed on a day to day basis by yourdesignated clinical lead, and will receive formal supervision from a B7therapist either within group or individual sessions.
To be supported by the designated Clinical Lead to carry outyour duties via informal supervision and guidance on an as needs basis,dependent on your level of experience and complexity of patients under yourcare.
To work within established Service / Trust policies andprocedures and contribute / propose changes for the development of operationalprocedures / working practices relevant to the team and services it provides.
To be responsible for providing formal clinical supervisionand support to lesser experienced therapists, plus Assistant Practitioners andCare support workers. This includes personal and professional development,including appraisal as well as support for case management beyond their level ofknowledge and experience. Informal support will also be given on a day to daybasis to assist with caseload management.
To provide supervision, education and assessment of therapystudents; this would be to a graduate standard and involve working with universitiesto ensure the standard of practice and teaching meets the standard set by thedegree level qualification
To undertake evidence based audit to further own and teamsclinical practice within the specialism. To make recommendations to theClinical Lead of service that may lead to implementation of specific changes topractice or contribute to service protocols
