Job summary

Hertfordshire Partnership University NHS Foundation Trust is seeking an enthusiastic Physiotherapist to join the Buckinghamshire Community Learning Disability Health Team on a part‑time basis to cover maternity leave.

The successful candidate will work with adults with learning disabilities across Buckinghamshire, visiting day centres, clinic spaces and homes while joining a multi-disciplinary team and developing the physiotherapy service in a community setting.

Qualifications

• Physiotherapy degree with HCPC registration and ongoing professional development.
• Experience in community-based working and learning disabilities is preferred.
• Experience in MDT working and managing own caseload.

Responsibilities

Job description

We are looking for an enthusiastic and creative Physiotherapist to work in our Buckinghamshire Community Learning Disability Health Team to cover maternity leave. This is a part‑time position for HCPC registered Physiotherapists with an interest in community LD services. The successful candidate will work as an integral member of a diverse multi-disciplinary team and will join the growing Physiotherapy service. This is a community role for a band 5 Physiotherapist and involves visiting service users at day centres, clinic spaces and their homes. A passion for working with adults with learning disabilities in this varied and interesting role is essential. The team has office bases in Aylesbury and High Wycombe. The role is county wide and we can adapt work locations, workdays and hours to support your circumstances. The role will involve managing your own caseload providing Physiotherapy assessment and treatment interventions for adults with a learning disability on our three main pathways, complex postural management, falls and mobility and community level respiratory care. Interventions delivered will include a range of individual treatment and group sessions. You will have a role in health promotion and be a key player in helping continually improve and develop the service. Our aim is to bring Physiotherapy to service users in a fun and engaging way and you will be able to use your creativity to do this., To provide Physiotherapy assessment and treatment interventions for a specified service user group, to carry a caseload as a member of a multi-disciplinary team and to develop the Physiotherapy service provision within the setting. Key Relationships

• To work with other team members of both the Physiotherapy team and wider MDT.
• To work with service users and carers within a variety of community-based settings across Buckinghamshire.
• To liaise with voluntary or other organisations as appropriate.

Duties and Responsibilities

• To manage a caseload, using evidence based/client principles to assess, plan, implement and evaluate interventions in a variety of community-based settings.
• To maintain clinical records.
• To develop skills and knowledge through participation in the Band 5 Physiotherapy Development Programme and competency framework.
• To gain experience through involvement in CPD activities and on the job learning opportunities.
• Clinical Responsibility: To plan and implement individual and/or group interventions, in collaboration with the service user, using appropriate interventions to achieve therapeutic goals.
• To undertake Physiotherapy assessments for a designated caseload, addressing all areas of complex physical health as appropriate with support where necessary.
• To work with service users to identify Physiotherapy goals as part of the overall care plan.
• To be responsible for the use of appropriate outcome measures in your clinical work to effectively evaluate physical outcomes for the service user and the Physiotherapy service.
• To assess service users understanding of interventions, gain valid consent and have the capacity to work within a legal framework with service users who lack capacity to consent to treatment.
• To maintain accurate and timely record keeping in line with legal, professional and Trust requirements.
• To carry out assessments and manual treatments of service users with moderate physical effort and to comply with the Trusts Manual Handling Policy at all times.

Professional Qualifications and Experience

• Physiotherapy Degree
• HCPC registration, Demonstrates a desire to work with people who have a learning disability, Documented evidence of continuing professional development.
• Some experience of community based working / mental health / learning disabilites / paediatrics
• Experience of MDT working
• Experience in managing own caseload, Full driving license with access to car unless you have a disability as defined by the Disability Discrimination Act 1995, Evidence of working alongside service users, often with complex manual handling, to support them in undertaking activities and treatments identified in their care plan.
• Demonstrates knoweldge of national and local polices around safeguarding
• Demonstrates knowledge of to gain valid consent and have the capacity to work within a legal framework with service users who lack capacity to consent to treatment.
• Computer literacy including basic proficiency in using Word, Excel, e-learning packages, Electronic Patient records and Powerpoint
• Strong written and verbal communication skills with the ability to adapt communication to people who have communication difficulties
• Demonstrates knowledge of basic respiratory assessment and treatment
• Demonstrates knowledge of basic falls awareness and treatment options
• Demonstrates knowledge of basic postural management, You must have appropriate UK professional registration.
