Sanctuary Personnel is seeking a locum Senior Physiotherapist in Bolton to support the Community Social Service team. The role focuses on comprehensive physiotherapy assessments, DTAH discharges, telecare and sensory services, and collaboration with hospitals and local partners.

The position offers a full-time, ongoing locum arrangement with a competitive hourly rate and flexible scheduling.

Responsabilidades

• Work alongside the independent living service team to provide comprehensive physiotherapy assessments and interventions.
• Facilitate smooth and effective patient discharges through the Discharge to Assess at Home (DTAH) protocol.
• Deliver exceptional telecare and sensory services as part of a holistic approach to community social services.
• Collaborate with hospitals, Bolton at Home, and other partners to enhance patient outcomes.
• Apply your knowledge and skills to tailor interventions that promote independent living and improve quality of life for our service users.
• Maintain accurate and up-to-date records in line with service protocols and HCPC guidelines.
• As a car driver, you will be able to travel across locations facilitating more effective and timely service delivery.
