Reablement - Empowering Independence

Join our Reablement Team and help people regain mobility, confidence, and independence after illness, injury, or hospital discharge.

What You'll Do
  1. Work with a wide range of conditions including neurological (e.g., stroke, Parkinson's), musculoskeletal (e.g., arthritis, fractures)
  2. Support patients in their homes and community settings
  3. Assess, treat, and adapt personalised care plans
  4. Collaborate with a multidisciplinary team
  5. Uphold safeguarding and clinical standards
What You'll Need
  1. Degree in Physiotherapy + HCPC registration
  2. 2+ years post-qualifying experience
  3. Ability to travel across Wiltshire
  4. Strong communication & interpersonal skills
  5. A proactive, compassionate approach
