Job description

Are you looking for a new challenge? Are you motivated to find a role where your development is encouraged? Do you have experience of working with older age service users with mental health needs and/or dementia? Are you passionate about providing a high quality service to older people, using your skills to support recovery and quality of life?

If yes, you could be the right person for our Physiotherapy vacancy. This role focuses on completing physiotherapy assessments and treatment plans for older people with mental health needs and/or dementia in a mental health inpatient unit. You will be supported by a senior physiotherapist but will be expected to work independently and manage/prioritise your caseload. You will receive clinical supervision, you will be expected to identify and complete CPD tasks proactively. You will be required to complete an appraisal and development plan with support.

The post is part of the Healthy Living Service which focusses on improving the physical health of service users in our inpatient units.

You will work as part of a multidisciplinary team to meet the needs of older people (age 65 and over) across 4 inpatient wards. The MDT includes AHPs (dietitian, SaLT and Ots), nursing, support staff, psychology and medics. You will be expected to take the physiotherapy lead at clinical handovers and MDT meetings to outline progress so far and future plans. You will also be expected to identify physical health needs of service users and escalate this to the medical team, working collaboratively to ensure the best physical health outcomes for the service users. You will work with the Health Living Advisors and Physiotherapy apprentice to identify physiotherapy needs and provide clinical guidance and risk management to support physical activity opportunities for service user recovery. You will need to work with trust colleagues and stakeholders within the community and NHS Trusts to support staff and efficient discharge plans. You will also be required to support admissions to the hospital and ensure physiotherapy assessment and care plans are safe, created in a timely manner and communicated effectively to the MDT. You will need to support service users that are physically unwell and are required to be transferred to the acute Trust ensuring the MDT are provided with updated care plans where necessary

Working as a physiotherapist within an older adult inpatient unit embedded within the MDT, providing coordinated recovery plans and condition management as part of an inpatient pathway. This can be from point of early diagnosis through to end of life care.

• To provide specialist holistic and physiotherapeutic assessment and advice/treatment of patients with diverse and complex physical, cognitive, psychological and neurological presentations in an inpatient setting. To develop and deliver individualised treatment programmes.
• To provide physiotherapeutic advice and support to other professions within the wider multidisciplinary team and external agencies. To act as a clinical resource, problem solving issues relating to care of older people.
• To undertake all aspects of clinical duties, which are within your scope of practice, as an autonomous practitioner within the older peoples service line.
• To hold responsibility for own caseload working without direct supervision. Supervision takes the form of regular 1:1 clinical and managerial sessions, formal training, clinical reasoning sessions, peer review, and multi-disciplinary team discussion..
• Make recommendations for changes to practice and may contribute to the implementation of specific changes to practice or service developments.
• Participate in student training as appropriate.
• Work in liaison with Healthy Living Service team and other multi professional staff in the service line.
• Promote the benefits of active lifestyles and link to improved mental health.
• To maintain clear links with your professional structures and your professions lead within the trust. Support the professions development and modernisation and the adherence to your professions standards and code of practice.
