  • Maintain annual HHRG values at the National Standard as appropriate.
  • Utilize HHRG values to establish visit frequency and interdisciplinary utilization.
  • Ensure annual outcomes meet National Benchmarks.
  • Refer patients to community resources as appropriate.
  • Evaluate the clinical needs of the patient; develop, implement, and revise the plan of care; document accordingly.
  • Participate in inter-disciplinary patient case conferences; evaluate the effectiveness of the care plan.
  • Supervise and instruct the Home Health Aide plan of care every 14 days with documented evidence.
  • Notify MD/other care providers of significant patient condition changes.
  • Complete admission and other necessary paperwork promptly.
  • Provide clear, concise narrative documentation reflecting skilled care, teaching, and patient/caregiver response.
  • Obtain and clarify valid MD orders for all care provided.
  • Plan and coordinate discharges, inform patient/MD, and document in the discharge summary; complete all discharge paperwork.

Consistently deliver service excellence to patients, families, visitors, volunteers, and colleagues, reflecting VNA’s CARES values: compassion, accountability, respect, excellence, and service.

  • Bachelor's or Master's Degree in Physical Therapy from an accredited institution.
  • Current PT license in Massachusetts, with at least one year of clinical experience and CPR certification.
  • Computer skills preferred.

Purpose of Position: Conduct comprehensive physical therapy assessments, considering physiological, psychosocial, environmental factors, and health history. Develop a plan of care to promote function and safety, and participate in policy and program development to enhance patient care.

Schedule Details: 37.5 hours/week, various shifts including some holidays and weekends. Area: Upper Cape. Sign-on Bonus Eligible!
