Job description

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a full-time opportunity performing home health visits for our senior living office located in Dover, DE. This office services adult and geriatric clients on a per-visit basis across territories in Dover, Smyrna, and Hartley.

As a home care Physical Therapist, you will be an integral part of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing, enabling clients to receive necessary medical care to remain at home.

Responsibilities include:

1. Make home visits in designated geographic territories.
2. Perform diagnostic tests and measurements, such as mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Implement integumentary protective interventions to prevent wound formation and promote healing using evidence-based dressings and topical agents.
4. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and balance interventions.
5. Assess and revise care plans regularly, collaborating with nursing staff as needed.
6. Educate clients and their families on rehabilitative care and activities to promote health, safety, and independence.
7. Document observations, interventions, and evaluations accurately using a touch pad tablet.

Qualifications:

• Current Delaware Physical Therapist (PT) license.
• At least one year of recent work experience as a PT.
• Graduation from an APTA-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
