Physical Therapist – Assisted Living Facility

Salary Range: $90,000 – $110,000 per year

Location: Portsmouth, Virginia

Job Type: Full-Time

Schedule: Monday to Friday, 9 AM – 5 PM

We are seeking a dedicated and licensed Physical Therapist to provide on-site therapy services in assisted living facilities. This position offers a meaningful opportunity to make a daily impact on the lives of older adults, supporting wellness, function, and fall prevention in a supportive, low-stress environment.

Our mission is to help residents maintain independence, mobility, and quality of life within their home environment.

Key Responsibilities:

  • Conduct thorough evaluations and create individualized care plans tailored to resident needs.
  • Provide direct therapeutic interventions focused on balance, mobility, strength, and ADLs.
  • Monitor progress and adjust treatment plans accordingly.
  • Educate residents and caregivers on exercise programs, safety strategies, and functional goals.
  • Collaborate with facility staff, physicians, and interdisciplinary care teams.
  • Graduate of an accredited Physical Therapy program (DPT or MPT).
  • Active Virginia Physical Therapist license (or eligibility to obtain).
  • Experience with geriatrics or senior living settings preferred.
  • Strong clinical, communication, and organizational skills.
