Responsibilities
- Provides physical therapy services under the supervision of a licensed physical therapist, following the physician-approved plan of care
- Observes and reports patient responses and condition changes to the supervising PT, physician, and clinical staff
- Promotes wellness and prevention, providing resources for future needs
- Documents supervisory activities in the patient’s medical record
- Monitors and documents patient responses to therapy
- Participates in care conferences, in-service trainings, audits, quality activities, and peer reviews
- Maintains confidentiality of patient information
- Recommends follow-up care and coordinates with other care providers and community resources
- Performs other duties as assigned
Qualifications
- Current license as a physical therapy assistant in the state of employment
- Graduate from an accredited PTA program (CAPTE)
- At least one year of physical therapy experience for those new to home health; less than one year requires approval and mentoring
- Current CPR certification
