Responsibilities

• Provides physical therapy services under the supervision of a licensed physical therapist, following the physician-approved plan of care
• Observes and reports patient responses and condition changes to the supervising PT, physician, and clinical staff
• Promotes wellness and prevention, providing resources for future needs
• Documents supervisory activities in the patient’s medical record
• Monitors and documents patient responses to therapy
• Participates in care conferences, in-service trainings, audits, quality activities, and peer reviews
• Maintains confidentiality of patient information
• Recommends follow-up care and coordinates with other care providers and community resources
• Performs other duties as assigned

Qualifications

• Current license as a physical therapy assistant in the state of employment
• Graduate from an accredited PTA program (CAPTE)
• At least one year of physical therapy experience for those new to home health; less than one year requires approval and mentoring
• Current CPR certification
