Job Overview

An opportunity has arisen for a part-time Physical Health Support Worker to join a vibrant Early Intervention Service on a permanent contract. The team covers the South and Central areas of Manchester and operates from Chorlton House.

The post holder will work alongside the Early Intervention Team, under the supervision of the Physical Health Lead and Team Manager. Responsibilities include undertaking physical health checks, clinics, and general health and wellbeing assessments. The role involves assessing physiological observations, promoting healthy lifestyles, and assisting service users in accessing support to recover from mental and physical health issues, aiming to promote recovery and facilitate return to daily living. The post also includes supporting care co-coordinators in planning, implementing, and evaluating care within care pathways as part of the stepped care model.

Main Duties
  • Conduct physiological assessments such as blood pressure, pulse, weight, BMI, and venepuncture for blood tests.
  • Assess service users holistically, including lifestyles such as exercise, diet, smoking, alcohol, and drug use; provide advice and signposting.
  • Work alongside the physical health nurse during clinics and community visits, participating in team activities.
  • Collaborate with team members to ensure a holistic, multi-disciplinary approach.
  • Contribute to the CPA process by liaising with care coordinators for needs assessments and reviews.
  • Enjoy pay enhancements: 30% additional for evenings (after 8 pm) and Saturdays; 60% additional for Sundays and Bank Holidays.

Qualifications
  • Recognised competency-based Level 3 NVQ or equivalent experience.
Experience
