Norfolk and Suffolk NHS Foundation Trust is seeking an experienced Community Matron to provide clinical leadership within the Central Norfolk Physical Health Team. The role entails governance across CMHTs, ensuring high standards of patient safety and quality of care.
You will work with community mental health teams to improve physical health outcomes for service users, collaborating with ICB and partner agencies to align services and drive continuous improvement.
We are pleased to advertise an exciting opportunity for an experienced and dedicated Community Matron to join our Physical Health Team in Central Norfolk. Our service works to local and National policy to increase parity of esteem and reduce health inequalities for service users with Serious Mental Illness.
As a community matron, you will provide direct supervision and governance to the physical health team and work alongside community mental health teams to improve the physical health of all service users in Norfolk. You will demonstrate sound clinical leadership and safe clinical decision-making and expert practice within a mental health setting.
The role also involves collaboration with the ICB and partner agencies to ensure the NSFT physical health offer aligns with system progression and joined-up working. Critical thinking, effective communication, and a commitment to delivering high-quality, person-centred care will be essential within this rewarding and dynamic role.
We are looking for an experienced Registered Nurse who can provide clinical leadership to inform evidence-based practice.
The successful candidate will be based with the Central Norfolk Physical Health Team at Hellesdon Hospital but will visit other CMHT sites in the county.
