Job description

We are looking for a confident Physical Health Lead Nurse (RGN) who can inspire, support, and oversee the delivery of excellent care.

You'll be working 40 hours a week (Monday to Friday, 9am - 5pm) at Cygnet Pindar House.

This post is supernumerary; you will not be included in the ward day-to-day numbers. We have a mix of RMN, RGN, and RNLD nurses who all contribute effectively to the excellent patient care we provide.

Cygnet Pindar House is our 22-bed Neuropsychiatric rehabilitation service for men affected by acquired brain injuries.

It is a safe and secure environment to aid recovery, as well as offering those diagnosed with progressive neurological diseases like Huntington's Disease a caring, longer-term placement to support and manage their symptoms' progression.

The service accepts unplanned, often urgent admissions where a service user requires a safe place to start treatment promptly, as well as planned admissions.

Your day-to-day

  1. Assess, plan, develop, implement, and evaluate programmes to promote health and well-being, and prevent adverse effects on health.
  2. Oversee and evaluate individual care and treatment plans for patients with physical health conditions.
  3. Identify and manage care and treatment plans for patients at risk of developing physical health conditions.
  4. Regular monitoring and interventions for patients with long-term conditions such as diabetes, asthma, hypertension, epilepsy, etc.
  5. Prioritize physical health problems and intervene appropriately, including in complex, urgent, or emergency situations, with effective emergency care.
  6. Support patients in adopting health promotion strategies and principles of self-care.
  7. Provide information and advice on physical health medication regimes, side effects, and interactions.
  8. Assess and manage patients with wounds.
  9. Work closely with visiting GPs overseeing referrals, health checks, and specialist services.
  10. Collaborate with ward doctors on phlebotomy and ECG services, ensuring accuracy and timeliness.
  11. Responsible for physical health induction and staff training.
  12. Implement hospital-wide smoking cessation programmes.
  13. Oversee referrals to dieticians, opticians, dentists, hearing tests, chiropody, and SALT services.
  14. Regularly check and order physical health supplies and equipment as needed.
