About the role
The local population in Hillingdon is diverse with a wide range of health and care needs. Working with health partners, NHS provider Trusts, GPs, and voluntary and community organizations, you will be supporting the delivery of a Population Health Management System that will provide a range of services including urgent and emergency care, mental health, primary care, elective hospital services and community care.

You will be working for the London Borough of Hillingdon as a member of the Population Health Management team that has responsibility for delivering four statutory ICB functions:
- To provide data and analytics to improve outcomes in population health and healthcare
- To provide data and analytics that identifies areas of inequalities in relation to access, outcome, and experience
- To enhance productivity through provision of digital tools and data
- To work in partnership with local authorities and voluntary sector organizations to provider broader social and economic analysis
You will be supporting a multi-faceted team comprising clinicians, administrations, relationship managers, programme, and project managers with the provision of high-quality data, analysis, and insight. The key elements of work can be summarised as follows.

About You
To be successful in this role, you'll need experience in:

Your success in this post will be achieved through partnership work across the council departments, NHS Partners, VCS, and wider health organisations. You will lead and support activity that will identify opportunities to align and embed a Population Health Management System across all relevant services.
This is a fixed term contract for 12 months.
