Summary: Pharmacy Technicians are vital contributors, working collaboratively with Clinical Pharmacists, Community Pharmacy teams, and other members of the general practice multi-disciplinary team. The role involves enhancing the development and delivery of services to the practice population through effective collaboration. The successful candidate will undertake a range of responsibilities, including clinical, technical, administrative, and communication tasks.

Key Responsibilities:

Clinical:

  • Conduct patient-facing and patient-supporting roles for effective medicines use through shared decision-making conversations.
  • Perform medicines optimization tasks, such as checking inhaler technique, supporting medication reviews, and medicines reconciliation.
  • Utilize consultation skills to partner with patients to ensure optimal medication use.
  • Support medication reviews and reconciliation for new care home patients and synchronize medicines for patient transfers between care settings.
  • Assist in Structured Medication Reviews (SMR) by organizing necessary monitoring tests.
  • Provide expertise for addressing public health and social care needs, offering lifestyle advice and tackling health inequalities.
  • Manage Shared Care Protocols and support the Clinical Pharmacist for more complex patients.
  • Contribute to antimicrobial stewardship initiatives to reduce inappropriate antibiotic prescribing.

Technical & Administrative:

  • Support the multi-disciplinary team in ensuring efficient medicines optimization processes.
  • Implement efficient ordering and return processes, reducing medication wastage.
  • Provide training and support on the legal, safe, and secure handling of medicines, including Electronic Prescription Service (EPS) implementation.
  • Promote Electronic Repeat Dispensing (eRD) and online ordering.
  • Develop relationships with pharmacy professionals and multi-disciplinary team members.
  • Support the practice in reviewing and developing policies for CQC requirements.

Communication:

  • Recognize the roles of colleagues within the organization and employ appropriate communication to gain cooperation.
  • Work collaboratively within a team, recognizing personal limitations and referring to more appropriate colleagues when necessary.
  • Develop and maintain effective working relationships within and outside the practice and locality.
  • Foster strong links with services across the locality, exploring opportunities for collaborative working.
  • Liaise with stakeholders on prescribing-related matters to ensure consistency of patient care and benefit.
