Pharmacy Technician Proactive Care and Frailty
The Pharmacy Technician is responsible for the following key duties in delivering health services:
- Undertake patient-facing and supporting roles to ensure effective medication use through shared decision-making conversations with patients.
- Perform medication optimisation tasks, including effective medication administration (e.g., checking inhaler technique), supporting medication reviews, and medicines reconciliation.
- Utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively, as required.
- Support medication reviews and medicines reconciliation for new care home patients and synchronise medicines during patient transfers between care settings, liaising with local community pharmacists.
- Provide specialist expertise, when competent, to address public health and social care needs, including lifestyle advice, service information, and tackling local health inequalities.
- Maintain a central role in the clinical aspects of shared care protocols and liaise with specialist pharmacists for complex cases.
- Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing.
- Assist in delivering medicines optimisation and management incentive schemes and conduct patient safety audits.
