Aneurin Bevan University Health Board is seeking a Pharmacy Technician to join the Grange University Hospital Pharmacy Department. The role offers opportunities to work across multiple specialties and to train while progressing toward ACT and Medicines Management qualifications.

You will support daily medicines management, aid in patient discharge planning, and contribute to an integrated medicines management service with robust CPD and professional development opportunities.

Responsibilities

Work with Senior Pharmacy Technicians and Pharmacists to support daily ward-based Medicines Management
Ensure medication is labelled for inpatient use and discharge
Assess patients' own medications for inpatient suitability and at discharge to prevent duplicates
Support medicines reconciliation by documenting medication history from multiple sources
Resolve discrepancies in medication history with ward pharmacist or clinician
Provide accurate information to patients, carers and healthcare professionals
Counsel patients on medicine use, considering understanding and emotional well-being
Coordinate with primary and secondary care to address discharge needs and avoid readmission
Perform final checking of dispensing by other staff after ABUHB ACPT reaccreditation
Train and assess other pharmacy technicians in this role

Job Overview

We are looking for a highly motivated Pharmacy Technician to become part of our supportive and forward-thinking team at The Grange University Hospital. As a valued member of our busy Pharmacy Department, you'll have the opportunity to expand your knowledge, develop your career, and make a meaningful impact on patient care while helping us shape the future of pharmacy services. Whilst ACT and Medicines Management qualifications are required for this post, we welcome applications from GPhC registered Pharmacy Technicians who wish to train and work towards these qualifications. During training, the contract will be Annex 21 of a Band 5 until qualifications are achieved. The Grange University Hospital is a Specialist Critical Care Hospital which provides the opportunity for our Pharmacy Technicians to work across a range of specialties including Intensive Care, Cardiology, Haematology, Surgery, and Paediatrics. Pharmacy enjoys excellent multidisciplinary links within the Health Board and we are committed to the professional development of our strong team. The Health Board has automated pharmacy systems and a comprehensive medicines management service including electronic discharge.

Main duties of the job

To work closely with the Senior Pharmacy Technicians and Pharmacists to assist in the daily running of the Medicines Management ward-based service.
To ensure the provision of medication suitably labelled for inpatient use and discharge.
To assess patient’s own medications for suitability of use as an inpatient and upon discharge to prevent duplicate supplies and support cost savings.
To support medicines reconciliation by confirming and documenting medication taken by patients prior to admission to hospital, using a range of sources.
To appropriately resolve any discrepancies or interventions involving the patient’s medication history and referring back to the ward pharmacist or appropriate healthcare professional where necessary.
To communicate accurate information to patients, carers, members of pharmacy and other healthcare professionals.
To provide information/counselling on the use of medicines to patients and/or carers, considering the level of understanding and emotional well‑being.
To liaise with other healthcare professionals within primary and secondary care with regards to pharmaceutical discharge needs of patients, particularly for patients who are at risk of readmission as a result of poor compliance.
To accurately final check the dispensing of other members of the department, following completion of ABUHB ACPT reaccreditation.
To undertake training and assessment of other pharmacy technicians training for this role.
