A leading healthcare provider in Bolton seeks a Band 4 Pharmacy Technician to join their Integrated Community Services team. The role involves collaborating with multidisciplinary teams to enhance medication management and support patients in their homes. Responsibilities include improving pharmaceutical services, conducting patient follow-ups, and providing medication advice. This position offers an opportunity to make a significant impact on patient care and medication safety.
An opportunity has arisen for a Band 4 Pharmacy Technician to join our Integrated Community Services (ICSD) Pharmacy team. The ICSD Pharmacy team plays a significant role in supporting patients following an acute hospital admission or to prevent avoidable admissions to hospital during periods of acute illness.
The role involves working under supervision as part of a multidisciplinary team, including GPs, Consultants, Nurses, Pharmacy, Support Workers, Therapists, independent providers, and colleagues within the Local Authority. We also collaborate with independent care providers, offering training around medication.
The objectives of the service are to support the delivery of safe, clinically effective, and cost-effective prescribing through medicines optimisation, waste minimisation, reduction in polypharmacy, and safeguarding issues related to medication. We work collaboratively across the healthcare system, including care homes, community services, CCGs, Bolton NHS Foundation Trust, social care, community pharmacy, families, and carers to support and optimise medicines use for residents and patients in their own homes. Our goal is to support individuals in reaching an appropriate level of independence with their medicines and to assist healthcare professionals in gaining insights into medicines management.
