This is a unique and rewarding opportunity for a motivated pharmacy technician to join our team in a split role between pharmacy clinical services (anticoagulation and cardiac services) and the hospital discharge teams.

As part of the anticoagulant team, the successful candidate will play a vital role in the Bristol Royal Infirmary's Warfarin Clinic. The post holder will provide specialist clinical advice and support to both patients directly and to hospital colleagues, contributing to safe and effective anticoagulation therapy.

In the discharge part of the role, the post holder will become a key contact for the Hospital Discharge Team and BRI Discharge Lounge, working to improve discharge pathways and patient flow from hospital to home or community care.

Main duties of the job

1. To provide a specialist technical and clinical service to the pharmacy-run anticoagulant dosing service for patients within the UHBW catchment area.

2. To provide specialist anticoagulant advice to healthcare professionals and to improve anticoagulation patient care at ward level.

3. Review the daily pathway discharge profile and facilitate resolution of any pharmacy-related issues to prevent delays to patient discharge.

Assist patients and carers in the Discharge Lounge with medication counselling and complex medication-related issues arising during the discharge process
